Illinois CCP eligibility pre-screen

The Community Care Program (CCP) pays for in-home care for Illinoisans 60 and older who qualify. This pre-screen mirrors the program's published tests so you can see where you stand before requesting the free state assessment.

Check CCP first: a two-minute pre-screen

Four questions based on the tests the Illinois Community Care Program (CCP) publishes. Answer for the person who needs care.

Free, and not an application. This pre-screen does not decide eligibility — only the program decides, through its free in-home assessment. CareDoor is not a state agency, and by Illinois law no one may charge you for help with a Medicaid application.

1. Is the person who needs care 60 or older?
2. Do they live in Illinois?
3. Which daily activities do they need hands-on help with?

Choose all that apply — this mirrors what the state's DON assessment measures.

4. Are their savings and countable assets at or below about $17,500?

Count bank accounts, investments, and extra property. The home they live in and one car generally do not count.
